Becoming a Provider

Who Can Apply?

Monkey Mind welcomes:

• Mental Performance Coaches — Certified professionals who help athletes with mental skills
• Licensed Therapists — Licensed mental health professionals (LCSW, LPC, PsyD, PhD)
• Executive Coaches — Professionals focused on leadership and business performance

Application Process

1. Click "Apply Now" on the For Providers page (or navigate to Become a Provider)

2. Complete the multi-step application form:

   Step 1 — Personal Info

   • Name, email, phone
   • Provider type(s) — you can select multiple

   Step 2 — Professional Details

   • Years of experience
   • Headline (a brief tagline for your profile)
   • Bio (detailed description of your approach)
   • Specialties (e.g., anxiety, confidence, visualization)
   • Sports expertise (if applicable)

   Step 3 — Credentials

   • License number and state (if licensed)
   • Upload supporting documents: license, certifications, insurance, degree

   Step 4 — Pricing

   • Set your default hourly rate (you can add more session types later)

   Step 5 — Profile

   • Choose a URL slug for your public profile (e.g., monkeymindathletes.com/providers/dr-smith)

3. Submit your application

What Happens After Applying?

• Your application goes to the Monkey Mind admin team for review
• You'll see a Pending status on your provider dashboard
• You'll receive an email notification when your application is approved or if additional information is needed
• Once approved, your profile goes live and you can start accepting bookings

If You're Rejected

You'll receive a notification explaining why. You can update your application and resubmit.
